Injury Report Form
Event Coordinators must report all serious injuries, especially those that require an emergency response. Please complete the attached (fillable) PDF form, save it on your computer, then email it to the Secretary of the Board ([email protected]), or to [email protected]. Other members observing a serious injury should also fill out the form and submit it to the Secretary of the Board.
