This form is for current Pickleball Club Members that need a replacement name tag. New members automatically receive a name tag during the online Member application process in CourtReserve.

Name tags are $8.00 each. After we receive your order, a $8.00 "Misc Charge" will be added to your CourtReserve account, which you'll need to pay the next time you login.
